Qualitative Evidence Synthesis: Methods and Approaches

Not every systematic review question is best answered by pooling quantitative effect sizes. Questions about patient experience, acceptability of an intervention, barriers to implementation, or how people make sense of a health condition are genuinely qualitative questions, and qualitative evidence synthesis provides a family of rigorous, systematic methods for answering them -- distinct from meta-analysis, but no less methodologically serious.

Why qualitative synthesis is not just "narrative review"

A common misconception treats qualitative evidence synthesis as simply a less rigorous alternative to meta-analysis, used when quantitative pooling is not possible. This misunderstands the method entirely. Qualitative evidence synthesis follows its own systematic, transparent, and reproducible methodology -- a registered protocol, an explicit and documented search strategy, defined eligibility criteria, quality appraisal of included qualitative studies, and a structured synthesis process -- it simply synthesizes qualitative rather than quantitative findings, using methods specifically designed for that purpose.

Thematic synthesis

Thematic synthesis is among the most widely used qualitative synthesis methods, involving line-by-line coding of findings across included qualitative studies, organizing codes into descriptive themes that stay close to the original studies' own language and findings, and then developing analytical themes that go beyond individual studies to generate new interpretive insight across the body of evidence. This method is particularly well suited to applied, practice-relevant questions where the goal is generating clear, actionable themes -- barriers to medication adherence, for instance, or factors shaping patient trust in a healthcare setting.

Meta-ethnography

Meta-ethnography takes a more interpretive approach, developed originally in educational research and widely adopted in health-related qualitative synthesis. Rather than simply aggregating themes, meta-ethnography seeks to translate concepts and findings from one included study into the terms of another, identifying where studies' findings genuinely agree (reciprocal translation), where they meaningfully conflict (refutational translation), and building a new "line of argument" synthesis that offers an interpretation transcending any single included study. This method suits more theoretically or conceptually oriented questions, where the goal is genuine interpretive synthesis rather than a practical list of themes.

Framework synthesis

Framework synthesis applies a pre-existing conceptual framework -- drawn from theory, from a prior study, or developed a priori by the review team -- as a structure for organizing and synthesizing findings across included qualitative studies. This method works well when a relevant theoretical framework already exists and the review's purpose is explicitly to test, refine, or apply that framework against a body of qualitative evidence, rather than to generate new themes or interpretations from scratch.

Quality appraisal of qualitative studies

CASP (Critical Appraisal Skills Programme) checklists are among the most widely used tools for appraising the quality of individual qualitative studies for inclusion, assessing domains including clarity of research aims, appropriateness of the qualitative methodology to the research question, rigor of data collection and analysis, and reflexivity regarding the researcher's own role and potential influence on findings. Unlike RoB 2 or ROBINS-I's more standardized bias-domain structure, qualitative appraisal tools generally support a more holistic judgment of methodological rigor, reflecting qualitative research's different epistemological foundations.

GRADE-CERQual for qualitative certainty

GRADE-CERQual (Confidence in the Evidence from Reviews of Qualitative research) adapts GRADE's certainty-rating logic specifically for qualitative evidence synthesis, assessing confidence in each individual review finding across four components: methodological limitations of contributing studies, relevance of those studies to the review question, coherence of the finding across studies, and adequacy of the data supporting it. Reporting CERQual assessments alongside your qualitative synthesis findings gives readers the same kind of calibrated confidence signal that GRADE provides for quantitative systematic reviews.

Reporting standards: ENTREQ

ENTREQ (Enhancing Transparency in Reporting the Synthesis of Qualitative research) is the primary reporting checklist for qualitative evidence synthesis, analogous to PRISMA for quantitative systematic reviews, covering expected reporting items across the research question, search strategy, study characteristics, appraisal method, and synthesis approach. Following ENTREQ explicitly, and citing it in your methods section, signals to reviewers familiar with qualitative synthesis that your reporting meets the field's specific transparency expectations.

Choosing a synthesis method for your question

Thematic synthesis suits practical, applied questions needing clear, actionable themes. Meta-ethnography suits more theoretically ambitious questions seeking genuine interpretive synthesis across studies. Framework synthesis suits questions where an existing theoretical structure already fits and needs testing or refinement against the evidence. The choice should be driven by your specific research question and purpose, specified in your protocol before synthesis begins, exactly as your choice of meta-analysis model should be specified in advance for a quantitative review.

Mixed-methods reviews combining both approaches

Some research questions genuinely benefit from combining quantitative and qualitative evidence within a single review -- a convergent design running both syntheses in parallel and integrating findings at the end, or a sequential design where qualitative findings help explain or contextualize quantitative results. The JBI methodology for mixed-methods systematic reviews provides one of the more developed frameworks for this combined approach, with specific guidance on how to structure integration between the two evidence types rather than simply presenting them side by side as separate, unconnected sections.

Sample size expectations differ from quantitative reviews

Qualitative evidence synthesis often includes considerably fewer primary studies than a quantitative meta-analysis might, since qualitative research questions are frequently narrower and the pool of directly relevant qualitative studies smaller. This is not, by itself, a methodological weakness -- qualitative synthesis methods are designed to extract deep, richly interpreted findings from a modest number of studies, rather than relying on large sample sizes for statistical power the way quantitative pooling does, and reviewers unfamiliar with qualitative methodology sometimes mistakenly apply quantitative expectations about total study count to a qualitative synthesis where those expectations, rooted in a different epistemological tradition entirely, do not actually meaningfully apply in the same way, given how differently the two research traditions define what a sufficient, trustworthy evidence base actually looks like, a distinction genuinely worth explaining directly and plainly to committee members, journal reviewers, or collaborators who come from a more purely quantitative research background and may be less familiar with qualitative methodology's own standards.
